Late registration
When conducting a late registration the applicant to be registered must be one year or older for a late registration to be processed.
A notarized affidavit by a Justice of the Peace or a person high standing in the community including a Community Leader or Toshao requesting a Late Registration of Births which should include the following:
- Name of Child
- Place of Birth
- Date of Birth
- Parent/Parents name
- If parents are not present during birth registration they must be
- Accounted for in affidavit
Any of the following may be requested as supporting documents:
- One (1) Passport Size Photo of the person
- Hospital Statement/Midwife Statement
- Clinic Card
- School Letter (Must carry Date of birth)
- Job Letter (Must state date of Birth)
- Name, Date, and Place of birth for all other Children/Siblings
- Copy of Identification (ID) Card
- Supporting Affidavit and Identification (ID) Card to be done by other relatives
- Marriage Certificate of parents
- Death Certificate
- Baptismal Certificate
