Late registration

When conducting a late registration the applicant to be registered must be one year or older for a late registration to be processed.

A notarized affidavit by a Justice of the Peace or a person high standing in the community including a Community Leader or Toshao requesting a Late Registration of Births which should include the following:

  • Name of Child
  • Place of Birth
  • Date of Birth
  • Parent/Parents name
  • If parents are not present during birth registration they must be
  • Accounted for in affidavit

Any of the following may be requested as supporting documents:

  • One (1) Passport Size Photo of the person
  • Hospital Statement/Midwife Statement
  • Clinic Card
  • School Letter (Must carry Date of birth)
  • Job Letter (Must state date of Birth)
  • Name, Date, and Place of birth for all other Children/Siblings
  • Copy of Identification (ID) Card
  • Supporting Affidavit and Identification (ID) Card to be done by other relatives
  • Marriage Certificate of parents
  • Death Certificate
  • Baptismal Certificate
